1) Speaking Event Inquiry Form 

This form starts the process. The inquiry will be answered based upon ministry schedule and availability. Please submit your inquiry as early as possible to secure Carol's availability.

 

2) Speaker Confirmation Form

This form is a binding agreement confirming your event.  You can complete this form after the Speaking Event Inquiry Form is evaluated and it is determined that Carol is available.

3) Event Detail Form

This form is completed after your event has been confirmed. On this form you can clarify important details about your event to insure that the event is a  success. Please complete this form at least 8 weeks before the date of your event.
